A full comprehensive assessment of health needs by specialist nurses following referral.

Individually tailored care packages for the most complex care needs in the community.

Management of appeals process for Continuing Health Care and Free Nursing Care (RNCC)

Who can use the service?

Patients from age 18 upwards with a Bath & North East Somerset GP. Covers all areas of health care including physical and sensory disability, learning disabilities, mental health services.

Care homes and care agencies.

Other professionals, families, voluntary agencies.

Covers patients including some placed out of county in specialist units.

Health and social services colleagues from both primary and secondary care.
